The company needs some documents from the shareholder in order to process his request for the issue of duplicate share certificates.
Firstly, an application letter with all the details of the shareholding is required. Secondly, an indemnity bond, an affidavit and a surety form need to be duly signed and notarized. Thirdly, your identity and address proofs need to be provided.
These documents need to be sent to the company or its Registrar and Transfer Agent (RTA) for obtaining fresh share certificates.

I am enclosing herewith the following documents in support of my application for issue of duplicate share certificates.
1. Self-attested copies of my PAN and Aadhar cards.
2. Indemnity bond, Affidavit and Surety Form - duly executed and notarized.
