A letter of declaration from a supplier can serve as supporting evidence for an application for origin certification of their products shipment.
The format of such a declaration may be available at the customs authority office. You need to check that. If no such form is available, then you can prepare your own letter with all the details.
Here is a sample of such a letter of declaration from a supplier for its client. The letter has to be printed on the letterhead of the supplier duly signed and dated.
