Any employee before leaving an organization permanently must give his resignation in writing either by a letter to the appropriate officer or by an email to the official id of the senior officer.
A supervisor too has to submit his written resignation to his boss, and serve the necessary notice period as required under the employment contract.
Here is an example of a resignation letter from the post of a supervisor. You can remove some of the lines if you do not want to add it to your resignation letter.

I have taken this decision because of my personal and family reasons. I would serve the notice period and complete all handover formalities duly by the end of the last working day.
I thank the management for giving me the opportunity to work for this esteemed organization. I have truly enjoyed working here, which helped me broaden my potential and outlook.
I am proud to say that my employment here has been thoroughly gratifying and rewarding. Your mentoring has always guided and encouraged me to do better. I am thankful to you for that.
