A resignation letter by a nurse does not have to contain anything special as compared to the resignation letters of any other positions.
The important thing about the resignation letter is the last date of working should be clearly mentioned. Otherwise, you should mention clearly that you are going to serve 30 days of notice period as required effective from the letter date.
Be polite in writing the resignation letter. There is no point in raising unnecessary acquisitions towards anybody unless there was any unethical incident concerning you personally. Do serve the required notice period. Leave peacefully. Move on with your career.
