If an employee wants to correct his/her basic details in the records of Provident Fund (PF) account, then that employee and his employer are required to submit a joint request to the concerned EPFO field office by submitting a Joint Declaration Form. (Manual Application)
The basic details of the PF member (i.e. the employee) that can be requested for correction are:1) Name of the employee,
2) Father/ Husband Name,
3) PF / EPS Account No.,
4) Date of Birth (DD/MM/YYYY),
5) Date of joining (DD/MM/YYYY),
6) Date of leaving (DD/MM/YYYY).
If any other details may wrongly be entered in the PF records, you can consult with your Employer or EPFO office, they will guide you to the appropriate procedure for rectifying the error.
In order to reduce paperwork and delay in processing the manual applications, EPFO has developed a Unified Portal where members can submit online requests to his/her employer at Member Interface in the Unified Portal for correction in the basic details. (Online Application)
The physical form to be submitted i.e. joint declaration form as prescribed by EPFO is provided hereunder. The format may change from time to time. Do check with the EPFO office or your employer before proceeding.

2) At the top left - click on "File" option - then click on "Download" option - choose 'Microsoft Word (.docx)'

ReplyDeleteYes, supporting documents as a proof of the accurate information should be enclosed viz. Passport copy, Gazetted notification copy for name change, etc. It varies depending upon your application for change in that particular details whether name or date of birth or address etc.

ReplyDelete(1) First of all, it would be advisable for you to consolidate your multiple PF accounts into one account only. You can visit the EPFO website and do this. You may have to coordinate with your current and previous employers for this. Or else visit the PF office nearest to your city and take guidance from them.
DeleteIf you aren't going to consolidate, you may face some issues with your PF accounts in future.
(2) If something has to be changed urgently and you want to submit the Joint declaration form for it, then you can submit the form through your current employer at the EPF office wherein the PF account is maintained. Or else directly visit the EPF office and submit the form. And for changing the details in previous PF accounts, likewise you will have to visit the concerned EPF offices and submit the forms.
That's why - in order to avoid a lengthy process - try to merge all the PF accounts into one.
Visit EPFO website for more information or consult with the Payroll officer or HR department of your company for guidance on the same.

ReplyDeleteYou can check with the concerned officer with your company, he can provide you with the appropriate form to sign and submit. If they don't cooperate, login into EPF employee official site and try to update your KYC there. After submitting online request, it will come for approval of the employer and they've to approve online. Just coordinate.

Fill in the joint declaration form, sign and take signature of employer as needed. And have it submitted at the EPFO office along with supporting documents. You can directly go to EPFO office and submit it or where you are working you can request the payroll or concerned officer who goes to EPF office for other works.

ReplyDeleteIf you're in employment somewhere, it may have gone to the Employer for approval. There's is a online portal for that. Please check with the payroll/ hr department of your company who is responsible for this.
DeleteOtherwise, it might be an internal process in PF office. Your application may have been forwarded to the appropriate officer for approval. In that case, you may have to wait for some more days.

I'd suggest you go directly to the PF office and meet the concerned officer and explain about your matter and ask the reasons for the delay. If your matter is not resolved within a week after that, lodge a complaint to the higher authority. Find out the higher authority's email id from EPFO website and also submit a written complaint at the PF office, addressing the complaint letter to the higher authority.

DeleteIf you had submitted JD form manually at the PF office then they would have given an Acknowledgement copy.
DeleteIf you had submitted JD form online at the PF portal then you must have received the acknowledgement number. Check the PF portal again to find the option to download the acknowledgement copy.
Otherwise if your employer is asking, give them the acknowledgement number and date of submission.
I sent Joint declaration with attested documents to company HR ( mumbai office) by post few days back, i want know hr department only take care of further process or send back me the documents with company seal ?

You can sign in at unified member portal of epfo. Wherein you will find the options "Manage" >> "KYC" >> "Aadhar". You can update Aadhar there. And in that website you can use "Manage" >> "Basic Details" >> "Name" option to change your name. It needs to be approved by your employer.
